Hospital Fee Schedule
The hospital or medical facility is billed for the total fee.
Hospital/Practitioner
Fee Schedule
The hospital or medical facility is billed for a percentage of the fee, and the practitioner is billed for a percentage of the fee.
Practitioner Fee Schedule
The practitioner is billed directly for the fee.
